Rigorous safety for digital innovation.
Clinical automations and digital health apps carry real and serious risk. Our certified experts provide independent Digital Clinical Safety risk management and DCB0160 oversight, ensuring your digital pathways are safe, compliant, and ready for frontline care.
Speak to a clinical safety officerIndependent oversight you can trust.
We are the NHS, which means we understand that deploying a clinical algorithm requires a fundamentally different level of rigour than a corporate IT project. If your trust is building its own digital health tools or clinical automations, you need objective, expert oversight.
Our Clinical Safety Officers (CSOs) work alongside your development teams to conduct thorough hazard logs, manage Information Governance (IG) requirements, and produce the mandatory safety cases required by national standards. For the automated CKD referral pathway across North Central London, this meant completing a full Data Protection Impact Assessment and achieving DCB0160 compliance before a single referral was processed.
What our clinical safety service covers
Every clinical safety engagement is tailored to the complexity, risk profile, and deployment timeline of your digital project. These are the four core deliverables our Clinical Safety Officers provide.
Independent DCB0160 compliance audits
Providing objective, expert assessments to ensure your clinical pathways meet mandatory national safety standards.
Clinical safety hazard logging and mitigation
Working alongside your teams to proactively identify, record, and mitigate potential clinical risks before deployment.
Information Governance (IG) and cyber assurance
Ensuring all patient data is handled securely, aligning with the strictest data protection and NHS governance frameworks.
Clinical Safety Case Report (CSCR) generation
Producing the comprehensive, mandatory documentation required to approve your digital health tools for frontline care.
Assuring safe, governance-approved automation across North Central London.
Assuring safe, governance-approved automation across North Central London
When TPHC’s Digital Productivity team set out to automate the CKD referral pathway across North Central London, clinical safety was embedded from the outset. The pathway processes sensitive patient data across multiple clinical systems — integrating NHS e-Referral Service with EMIS to manage registration, triage, and discharge — creating real risk if not governed correctly.
Our Clinical Safety Officers led the governance work required to operate safely at scale. A full Data Protection Impact Assessment and compliance with the DCB0160 clinical safety standard were both completed before a single referral was processed. This gave all stakeholders — clinical leads, IG teams, and trust leadership — confidence that the automation was safe, sustainable, and compliant. The automation has run continuously since April 2024, processing 4,289 CKD referrals end-to-end to March 2026 without a clinical safety incident. It stands as a transferable reference model for other specialities considering comparable automation across NCL or beyond.
