Building the five-year investment case to reduce inequalities in long-term conditions care across north east London

We were commissioned by North East London Health and Care Partnership to support a system-wide approach to Local Enhanced Schemes in general practice, addressing differences in investment and access across north east London
Our initial assessment found that reviewing individual schemes would not make the best use of limited resources. Working with the client, we reframed the programme around a Long-Term Conditions Outcomes Framework for primary care across north east London. We developed the supporting business case, financial model and activity model to address historic differences in investment and reduce health inequalities.
The impact of our work
Working with senior commissioners and clinical directors, we developed financial business cases for five-year ICB investment. The cases demonstrated commercial, financial and economic viability, potential cost efficiencies and forecast improvements in health outcomes.
