Community Mental Health Transformation for Older Adults

The Community Mental Health Framework for Adults and Older Adults sets out ambitious changes for how older people living with complex mental illness can be supported live well within their communities. Transformation Partners in Health and Care (TPHC) is helping Integrated Care Systems (ICSs) in London to develop robust systems of community mental health care for older people.

London Older Adults Clinical Group

TPHC has established a working group of older adult clinical and operational specialists to collaborate on community transformation across London. The group is a space for the sharing of different perspectives, knowledge and expertise. Members are able to find collective challenges and work through some of the most complex barriers in their community transformation. The group focus on developing projects, guidance and resources around priority themes for London, including:

  • Meaningful coproduction and involvement with older people
  • Delivering a needs-led community MH service for older people
  • Generating and sharing data and information to support community MH transformation
  • Adapting outcome measures and care planning tools for older adult services
