Transformation Partners in Health and Care has launched a new toolkit designed to support the evaluation of social prescribing services.
The Social Prescribing Evaluation Toolkit, which has been co-produced with social prescribers and social prescribing managers from across London, is designed to provide guidance on how to gather data and measure the impact of local social prescribing services.
The online resource includes a wealth of local examples from across London where the impact of social prescribing has been successfully demonstrated, and the steps taken that enabled this to be achieved. This includes:
- North East London’s approach to measuring social prescribing impact – which included developing a clinical template for social prescribers and an ICS wide dashboard
- Barnet Social Prescribing Service’s quarterly and annual reporting – which combines GP, social prescribing and VCSE data to inform commissioning
- North West London’s approach to monitoring and recording social prescribing data through the adoption of a caseload management tool
- Local evaluations of social prescribing services in Merton and Bromley
It also provides practical guidance to addressing some of the key challenges that come with demonstrating the impact of social prescribing services, as well as links to resources including training and further case studies.
