Welcome to edition 7 of Insight.
Almost three weeks on from the long-awaited launch of the Government’s 10 Year Health Plan for England, the three headline shifts—analogue to digital, acute to community, and treat to prevent—are already setting the tone for what’s to come.
As I shared in my recent blog, this plan isn’t just about fixing what’s broken—it’s about reimagining care for the future. We’re here to support health and care colleagues to meet that challenge by sharing proven solutions that can be scaled and sustained.
Our recent partnership with North Central London Integrated Care Board and North London Councils, which earned us an HSJ Digital Award last month, is a powerful example of what’s possible:
Analogue to digital – New technology in care homes led to 36% fewer night-time checks and a 52% increase in restful sleep for residents.
Acute to community – We helped reduce avoidable hospital admissions, enabling more people to stay in their own beds, whether at home or in care homes.
Treat to prevent – In one care home, we helped reduce falls from 9% to 0% in just four months.

Celebrating a HSJ Digital Awards 2025 win with North Central London ICB and North London Councils

North Central London Integrated Care Board in partnership with North London Councils and Transformation Partners in Health and Care are proud to announce that we have been named a winner in the Connecting Health and Social Care through Digital category at the HSJ Digital Awards 2025, recognising outstanding achievements in harnessing digital technology to improve healthcare outcomes across the UK.
Our project focused on improving access to technology for adult social care services in north central London, supporting connection with their NHS colleagues, and rolling out remote and acoustic monitoring for falls prevention in care homes
This work has delivered significant benefits for care home residents and staff, with:
- over 1,000 residents receiving benefits such as early detection of illness or deterioration and reduced time spent in hospital.
- care homes with remote monitoring experiencing a 28% reduction in ambulance call-outs and a 15.7% reduction in avoidable admissions
- care homes with acoustic monitoring tools requiring 36% fewer physical checks, leading to 52% more restful sleep and helping to reduce falls.

Over the coming months we will continue to explore priority topics covered in the 10 Year Health Plan.
What it means for children and young people
Tooth extraction is the leading cause of hospital admissions among children aged 5 to 9 years old, yet it is almost entirely preventable. To address this, the 10-year plan sets out a focus on dentistry and its commitment to improving access to dental services.
We’ve been working with North Central London Integrated Care board to deliver a dental transformation programme that increases access to dental care in areas of higher deprivation and helps children and young people to better understand how to brush and look after their teeth.
How automation can help solve the NHS productivity crisis
The NHS can no longer afford to wait, and with the right tools it doesn’t have to. James Davies, Head of Digital Productivity at TPHC urges the NHS to adopt more automation now to boost productivity and achieve the three strategic shifts of the 10-Year Plan.

Join TPHC communications and engagement experts at an upcoming webinar on major service change and reconfiguration

TPHC’s communications and engagement team will be sharing learnings from their extensive work on the London and South East Children’s Cancer Centre reconfiguration. This webinar is hosted by NHS England’s Major Service Change Learning Network and is designed for NHS colleagues involved in major service change and reconfiguration.
Joining colleagues from NHS England, the team will reflect on how they worked in partnership to lead and deliver strategic communications and engagement for this programme for more than two years.
