This is the second edition of Transformation Partners in Health and Care’s Annual Impact report and provides a snapshot of our activity and impact in 2024/25.
We are proud to have worked alongside our partners and clients to navigate much change in the health and care system and support them to take important steps to achieve their transformation and improvement goals.

I am delighted to introduce our second Annual Impact Report for Transformation Partners in Health and Care (TPHC).
Over the past year, our outstanding Consulting, Partnerships, and Digital Productivity teams have continued to impress our clients with their exceptional talent, resilience, and ability to deliver meaningful change. Their commitment to NHS values has remained steadfast and, despite one of the most challenging years the NHS has faced, we are proud to have made a significant difference for both our clients and the people who rely on health and care services.
2024/25 was a time of considerable change and anticipation, with a new Labour government, a new Secretary of State for Health and Care, and the publication of the Darzi report, which was followed up in July 2025 by the publication of the 10-Year Health Plan for England. The NHS is embarking on a period of radical transformation, including three key shifts that will shape where our skills and support are most needed in the years to come:
- sickness to prevention
- analogue to digital
- acute to community
This report demonstrates how TPHC is already contributing to these shifts, with real-world examples of the impact we’re delivering for our partners and clients. It also showcases the breadth of knowledge and expertise across our teams — spanning data and technology, productivity, patient safety, mental health, primary care, large-scale change, and tackling health inequalities.
As with last year, uncertainty and pressure remain constant across the health and care system. TPHC is here to navigate these complexities with you, working in partnership to address challenges and deliver sustainable solutions that improve outcomes for all.
