Project shortlisted for Connecting Health and Social Care through Digital category at the HSJ Digital Awards 2025.

North Central London Integrated Care Board (NCL ICB) in partnership with North London Councils and Transformation Partners in Health and Care (TPHC) are thrilled to announce our shortlisting in the Connecting Health and Social Care through Digital category at the HSJ Digital Awards 2025, a recognition of its dedication to pioneering digital solutions that are shaping the future of healthcare across the UK.
This year marks a milestone for the HSJ Digital Awards, with a record-breaking 432 entries submitted. Of which, 205 projects have been shortlisted, each demonstrating remarkable ingenuity, passion, and transformative impact in digital health.
The HSJ Digital Awards shine a light on teams and organisations driving meaningful change through technology, improving patient outcomes, streamlining processes, and enhancing the overall quality of care. Being shortlisted is a testament to both of our organisations’ unwavering commitment to innovation and excellence in digital healthcare.
Digitising NCL adult social care homes
This project focused on NCL ICBs work to improve access to technology for adult social care services, supporting connection with their NHS colleagues, and rolling out remote and acoustic monitoring for falls prevention in care homes.
Care home residents are among the most vulnerable groups in society with complex health and social care needs, including frailty, dementia and multiple comorbidities, contributing to an elevated fall risk. Acoustic monitoring alerts professionals to patients at risk of falls, reducing the need for non-essential hourly checks and disruptions to residents’ sleep.
