NHS ConfedExpo 2025 takes place in under two weeks and TPHC Consulting and Partnerships colleagues are leading agenda sessions on day one of the conference.

Expectations vs actions: the AAR approach to patient safety and continuous improvement
TPHC’s Consultancy team are leading an interactive session on the role of After Action Review (AAR) in improving patient safety and continuous improvement. After Action Review (AAR) features in the new Patient Safety Incident Response Framework (PSIRF) as one of the key learning tools.
Delivered by Cassie Howe, Senior Consultant and Co-Lead of TPHC’s AAR Faculty and Sue Hunter, TPHC Managing Director and trained AAR Conductor, this interactive session offers participants an opportunity to consider how AAR uses past experience to inform future action.
They will be joined by system leaders Finola Devaney and Dr Giles Thorpe, who will share their experiences of implementing AAR in their organisations through a fireside chat, offering reflections from their experience of doing AARs and their impact so far.
The session is delivered by:

Cassie Howe, Senior Management Consultant – Transformation Partners in Health and Care

Sue Hunter MCMI ChMC, Managing Director – Transformation Partners in Health and Care (TPHC)

Finola Devaney, Director of Clinical Quality & Governance & Assistant Chief Nurse (Quality) – Princess Alexandra Hospital Trust

Dr Giles Thorpe, Executive Chief Nursing Officer – NHS Mid and South Essex
The session is designed to offer participants an introduction to AAR, and to consider how AAR uses past experience to inform future action.
