Transition to adult services

Moving from children and young people’s asthma services to adult services is called transition. This period should include a time to prepare, transfer of care to adult service and continues until the young person has engaged with the adult service. It may start from age 12 onwards and last until 25 depending on the child and / or condition. The process of transition is expected to take longer where a child has multiple, complex needs, but the key feature of transition is that care should remain flexible at all time.

Asthma + Lung UK, My Lungs My Life, How the NHS works for young people, a blog about “owning it” and 11 to 25 hub all have useful information for young people and their families about transition to adult services.
