Why we are transitioning
The team, working alongside NHSE London, ICB teams and other partners, have achieved success in delivering the personalised care NHS Long Term Plan (2019) targets across London. This includes:
- Enabling and supporting the growth of social prescribers in London from 120 in 2020 to ~500 in 2024
- Exceeding the 23/24 trajectories for Social Prescribing Link Workers, social prescribing referrals, and number of Personalised Care and Support Plans (PCSPs)
- Being the top region nationally for the number of residents supported by SP interventions in in every year of the LTP
Alongside specific LTP targets, the programme also supported many wider community led prevention initiatives, including proactive, personalised care approaches that connect patients to appropriate community-based support to tackle the social determinants of health.
Given the new London wide strategies, the development of Neighbourhood heath agenda and the government’s main health priorities relating to the shift from acute to community and treatment to prevention the strategic aims of the programme are now a core focus in health and care policy.
The programme activities are therefore being embedded within local delivery plans at ICB, Place and Neighbourhood levels. The regional programme has focused Q3 and Q4 of 24/25 completing and closing projects and transitioning BAU activities into NHSE London, ICBs and other system partners following guidance from The London Leaders Group and the London Regional Exec Team.
We have worked with ICB leads across London to support them with developing the infrastructure within their systems to continue to deliver the programme ambitions in the best way possible. The programme transition has had oversight from ICB Personalised Care Leads, London Leaders Group and NHSE London Regional Exec Team and Personalised Care team.

In addition, to ensure the continued legacy, all programme outputs have been transferred to NHSE London Region for ongoing management.
Ongoing regional support
Whilst most of the programme activities are being embedded with ICB delivery plans, there are several functions which will continue to be supported at a London Regional level from April 1st, 2025. These include:
- Bromley by Bow Insights, with support from NHSE London primary care team to support the continuation of the London Social Prescribing Manager Network
- NHSE London (Health Equity & Personalised Care Team) to support the continuation of the Secondary Care prevention Community of Practice by integrating with the wider personalised care Community of Practice
- The National Association of Social Prescribing (NASP) have initiated an ICB Social Prescribing Community of Practice nationally – whilst not London-specific, provides a forum for ICBs to get together and share learnings
- Clinical Leadership to continue with Dr Jagan John – NHSE London Clinical Director for Personalised Care
- Community Chest activity transitioned to the specific boroughs where these initiatives are continuing into next year – Stronger partnership working with VCSE sector embedded within various other workstreams across ICBs
- Development of the Social Welfare Legal Advice agenda will continue to be supported by the team at Bromley by Bow Insights
- NHSE London Region will establish a personalised care and health inequalities Community of Practice for ICBs and other system partners across London
